Pharmaceutical form: Sustained-release tablets 20mg.
Composition: The active ingredient of Nifelat R is Nifedipine.
Pink-grey, round film-coated tablets.
Nifelat R is a calcium antagonist with peripheral and coronary vasoditative properties. Nifelat R increases coronary blood flow and decreases myocardial oxygen consumption. Nifelat R has no anti-arrhythmic activity. Nifelat R tablets taken twice daily provide 24 hour control of increased blood pressure. The percentage reduction of blood pressure caused by Nifelat R is directly related to its initial height. Nifelat R does not reduce the blood pressure of normotensive individuals.
Nifelat R is indicated for the prophylaxis of angina pectoris and the treatment of hypertension.
The recommended dose of Nifelat R is one tablet two times daily taken with a little water during or after meals. If necessary the dose may be increased to two tablets two times daily.
The most common side-effects of Nifelat R are associated with the vasodilatory action, such as dizziness, headache, flushing, hypotension and peripheral oedema. A paradoxical increase in ischaemic chest pain may occur at the start of treatment. There have been reports of abnormalities in liver function due to hypersensitivity reactions, and gingival hyperplasia has been reported. Other loss commonly related side-effects include, rash. nausea. lethargy and increased frequency of micturition
Nifelat R should be used with caution in patients with hypotension and in patients whose cardiac reserve is poor. In patients with severe aortic stenosis Nifelat R may increase the risk of developing heart failure.
Nifelat R may enhance the antihypertensive effects of beta-adrenoceptor blocking agents although the combination is generally well tolerated. The use of Nifelat R in diabetic patients may require adjustment of their control.
Nifelat R should be discontinued in patients who experience ischaemic pain following its administration.
The antihypertensive effect of Nifelat R may be potentiated by concurrent administration of Cimetidine. When used in combination with Nifelat R,serum Quinidine levels have been reported to be suppressed.
Nifelat R is not recommended for use in children.
Nifelat R is contra-indicated in patients hypersensitive to Nifedipine. Nifelat R should not be administered to patients with cardiogenic shock.
Nifelat R is contra-indicated in pregnant women or women who may become pregnant.
Nifelat R should not be administered to nursing mothers.
In overdosage by mouth the stomach should be emptied by aspiration and lavage. Hypotension may be treated by placing the patient in the supine position with the feet raised. Standard rneasures, such as Atropine for bradycardia and Noradrenaline for hypotension, have been suggested if necessary. It has been also suggested that Calcium Gluconate combined with Metaraminol may be of benefit
Nifelat R tablets should be stored below 25.C, protected from light and moisture.
Do not use beyond the expiry date stated on container.
